  1. Non-critical: The ceiling tile in the facility need replaced, they are in poor condition. The floor in the back room is missing in some sections and is not easily cleanable.
  2. Non-critical: A cell phone was observed on the food prep area. Please keep these in a designated area away from food prep to reduce the contamination of bacteria.

The Wall Street Pub and Grille, 235 Miami St., Tiffin. Inspected Jan. 5, 2023:

  1. Non-critical: Physical facilities not maintained in good repair. THE WALL BEHIND THE MEAT SLICER/PEPSI REACH-IN-COOLER IS DAMAGED AND NEEDS TO BE REPAIRED.

Pit Stop, 321 E. Perry St., Tiffin. Inspected Jan. 5, 2023:

  1. Non-critical: A food packaged in the facility not labeled. THE BOLOGNA SANDWICHES IN THE SANDWICH REACH-IN-COOLER WERE NOT LABELED WITH INGREDIENTS OR MANUFACTURER INFO.

Rock Run Bulk Foods Catering, 2685 W. US 224, Tiffin. Inspected Jan. 9, 2023:

  1. Non-critical: No towels or drying device at the handwashing sink(s). PAPER TOWELS NEED TO BE PROVIDED AT ALL OF THE HANDWASHING SINKS.
  2. Non-critical: Physical facilities not maintained in good repair. IN THE BACK STORAGE AREA/HALLWAY, THERE WAS A STRIP OF INSULATION HANGING DOWN OVER COFFE DISPENSERS AND FOOD EQUIPMENT. TO PREVENT CONTAMINATION, THE INSULATION NEEDS TO BE FIXED SO THAT IT IS NOT HANGING OVER THE FOOD EQUIPMENT.

The Chandelier Community Event Center, 151 S. Washington St., Tiffin. Inspected Jan. 12, 2023:

  1. Non-critical: Single-service and single-use articles not protected from contamination. A PACKAGE OF CUPS WAS SITTING ON THE FLOOR IN THE STORAGE AREA. TO PREVENT CONTAMINATION, THESE NEED TO BE STORED OFF OF THE FLOOR.

Heminger Center at Tiffin University, 350 Miami St., Tiffin. Inspected Jan. 12, 2023:

  1. Non-critical: Facility does not have an employee with manager certification in food protection. SOMEONE WILL NEED TO GET THE MANAGERS CERTIFICATION IN FOOD SAFETY.
  2. Non-critical: Food thermometer not readily accessible. A FOOD PROBE THERMOMETER NEEDS TO BE PROVIDED.
  3. Non-critical: Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. THE HELLO GOODNESS REACH-IN-COOLER DOES NOT HAVE A THERMOMETER INSIDE.
  4. Non-critical: No sanitizer test kit available. QUAT TEST STRIPS NEED TO BE PROVIDED FOR THE SANITIZER AT THE 3-BIN SINK.

G & L Shell, 2942 S. SR 100, Tiffin. Inspected Jan. 17, 2023:

  1. Non-critical: Single-service and single-use articles not protected from contamination. IN THE BACK STORAGE AREA, 2 BOXES OF INSULATED CUPS WAS SITTING ON THE FLOOR. TO PREVENT CONTAMINATION, THESE NEED TO BE OFF OF THE FLOOR.

T.J. Willie’s, 738 W. Market St., Tiffin. Inspected Jan. 20, 2023:

  1. Non-critical: Ground beef was observed thawing out at room temp. To prevent bacteria growth thawing should be done in the W/C, in the cooking process or under continuous running water.
  2. Non-critical: Food in the W/C was not covered and two boxes of potatoes were observed on the floor. Food needs to be covered and stored off the floor to prevent contamination from dust and dirt in the facility.

Stiney’s, 255 Second Avenue, Tiffin. Inspected Jan. 20, 2023:

  1. Non-critical, repeat violation: No paper towels were available at the hand wash sink. Paper towels are needed to properly wash hands.
  2. Non-critical, repeat violation: The oven and fryer area need cleaned. Dirt and grease were observed on them during the inspection.
  3. Non-critical: The wall behind the fryer needs cleaned, grease and dirt were observed on it during the inspection.

Golden Crown, 126 Dwight St., Tiffin. Inspected Jan. 26, 2023:

  1. Non-critical: Utensils were observed not sanitized after being washed. All utensils and dishes need to be sanitized to prevent contamination of other food.
  2. Non-critical: Door gasket needs replaced on the standup cooler. This will help keep temps within acceptable range and save on electric costs.
  3. Non-critical: Equipment needs cleaned to prevent bacteria growth in the facility. Clean equipment door fronts and handles.
  4. Non-critical, repeat violation: Cleaning needs to be done to prevent growth of bacteria that could contaminate food.

Little Caesars, 318 W. Market St., Tiffin. Inspected Jan. 27, 2023:

  1. Non-critical: Non-food contact surfaces of equipment are unclean. THE INSIDE OF THE PREP COOLER AND THE PIZZA CONVEYOR/OVEN NEED TO BE CLEANED.
  2. Non-critical: Facility not maintained clean. THE FOLLOWING NEEDS TO BE CLEANED: THE FLOOR AROUND THE DOUGH MIXER, THE FLOORS THROUGHOUT, AND THE DISHWASHING AREA.
  3. Non-critical: Physical facilities not maintained in good repair. BEHIND THE DOUGH MIXER, THE WALL IS DAMAGED AND THERE IS MISSING FLOOR TILES.

Circle K, 975 W. Market St., Tiffin. Inspected Jan. 30, 2023:

  1. Non-critical: Non-food contact surfaces of equipment are unclean. THE WALK-IN-FREEZER FLOOR NEEDS CLEANED AND THE INSIDE OF THE TRUE UNDERCOUNTER COOLER (BY THE PREP/OVEN AREA) NEEDS TO BE CLEANED.

Tim Hortons, 1630 W. Market St., Tiffin. Inspected Jan. 31, 2023:

  1. Non-critical: Single-service and single-use articles not protected from contamination. STACKS OF CLEAR LIDS, BESIDE THE CAPPUCCINO MACHINE, HAD SPLATTER/RESIDUE ON THEM. THESE NEED TO BE MOVED OR PROTECTED FROM CONTAMINATION.
  2. Non-critical: Irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations. EITHER AN IRREVERSIBLE MAXIMUM REGISTERING THERMOMETER OR THERMOLABELS NEED TO BE PROVIDED FOR THE DISHMACHINE.

MST Pub & Grub, 92 Madison St., Tiffin. Inspected Jan. 31, 2023:

  1. Non-critical, repeat violation: Spray bottles need to be labeled when the product in them is not in the original container.
  2. Non-critical: The floor under the fridge and dry storage rack needs broomed. Please clean floors to both sight and touch.

Marco’s Pizza, 75 Melmore St., Tiffin. Inspected Feb. 6, 2023:

  1. Non-critical: Non-food contact surfaces of equipment are unclean. THE INSIDE OF THE DOUGH REACH-IN-COOLER NEEDS TO BE CLEANED.
  2. Non-critical: Facility not maintained clean. THE WALLS NEED TO BE CLEANED THROUGHOUT.

Restaurants with at least one critical violation:

Tiffin Developmental Center (Main Kitchen), 600 N. River Rd., Tiffin. Inspected Dec. 20, 2022:

  1. Critical: Refrigerated, ready-to-eat, TCS foods not properly date marked. READY-TO-EAT FOODS IN THE WALK-IN-COOLER WERE NOT DATEMARKED. THESE NEED TO BE DATEMARKED WITH A 7 DAY USE-BY DATE ONCE THEY ARE PREPARED AND/OR OPENED. THEY WERE GOING TO BE DATEMARKED.
  2. Non-critical: Facility does not have an employee with manager certification in food protection. NO ONE HAS THE FOOD SAFETY MANAGERS CERTIFICATION FROM OHIO DEPARTMENT OF HEALTH. BRENDON WILL BE TAKING THE CLASS AND TEST AFTER THE FIRST OF THE YEAR.

Shake Shak, 179 N. Washington St., Tiffin. Inspected Jan. 5, 2023:

  1. Critical: Gloves need to be used when preparing sandwiches that are ready to eat. This is to prevent contamination of ready to eat food.
  2. Non-critical: Box of cups and chips need to be placed 6 inches off the floor to protect them from contamination.

Penn Station, 596 W. Market St., Tiffin. Inspected Jan. 9, 2023:

  1. Critical, corrected during inspection: AN OPEN BOTTLE OF GLASS CLEANER WAS SITTING ON A PREP COOLER. THERE WAS NOT ANY LID ON THE BOTTLE. TO PREVENT CONTAMINATION, THE LID NEEDS TO BE KEPT ON ALL CHEMICAL/CLEANER BOTTLES. THE BOTTLE WAS MOVED.
  2. Non-critical, repeat violation: TIME AND TEMPERATURE LOGS ARE NOT BEING KEPT FOR THE FRENCH FRIES.

Tiffin Eagles Club FOE #402, 68 Riverside Dr., Tiffin. Inspected  Jan. 11, 2023:

  1. Critical: Refrigerated, ready-to-eat, TCS foods not properly date marked. Food that exceeds the 7 days needs thrown away. A pot of soup was discarded.
  2. Non-critical, repeat violation: Non-food contact surface(s) not cleaned at the required frequency. The hood above the fryer needs cleaned. Please clean frequently so the hood is operating properly.

Fort Ball Pizza Palace (South Store), 111 Melmore St., Tiffin. Inspected Jan. 11, 2023:

  1. Critical, repeat violation: Food not properly protected from contamination by separation, packaging, and segregation. OBSERVED CONTAINERS/PANS OF FOOD THAT WERE NOT COVERED. TO PREVENT CONTAMINATION, FOOD NEEDS TO BE KEPT COVERED.
  2. Critical, repeat violation: Refrigerated, ready-to-eat, TCS foods not properly date marked. OBSERVED CONTAINERS OF READY-TO-EAT FOODS IN THE WALK-IN-COOLER AND PREP COOLER THAT WERE NOT DATE MARKED. THESE NEED TO BE DATE MARKED WITH A 7 DAY USE-BY DATE.
  3. Non-critical: Single-service and single-use articles not protected from contamination. IN THE BACK ROOM, THE ENDS OF PIZZA BOXES ARE SITTING ON THE FLOOR. TO PREVENT CONTAMINATION, THESE NEED TO BE OFF OF THE FLOOR.
  4. Non-critical: Physical facilities not maintained in good repair. THE SHELVES IN THE WALK-IN-COOLER ARE GETTING RUSTY. THESE NEED TO BE REPLACED OR REFINISHED SO THAT THEY ARE SMOOTH AND CLEANABLE.
  5. Non-critical: Facility not maintained clean. THE FOLLOWING NEEDS CLEANED: THE OVEN VENTS/HOOD, THE WALL BEHIND THE PREP COOLER, THE WALL BEHIND THE PREP TABLE IN THE BACK ROOM AND THE INSIDE DOOR OF THE WALK-IN-COOLER.

Hunan King, 253 S. Washington St., Tiffin. Inspected Jan. 17, 2023:

  1. Critical: Food not properly protected from contamination by separation, packaging, and segregation. IN THE REACH-IN-COOLER, RAW BEEF WAS SITTING ABOVE READY-TO-EAT FOODS. TO PREVENT CROSS-CONTAMINATION, THE BEEF NEEDS TO BE STORED BELOW THE COOKED, READY-TO-EAT FOODS.
  2. Critical, repeat violation: Food not properly protected from contamination by separation, packaging, and segregation. OBSERVED FOOD CONTAINERS (SUGAR AND FLOUR BINS, IN THE WALK-IN-COOLER, AND SITTING IN THE KITCHEN/PREP AREA) THAT WERE NOT COVERED. TO PREVENT CONTAMINATION, THESE NEED TO BE KEPT COVERED.
  3. Critical: TCS foods not being cold held at the proper temperature. LARGE CONTAINERS OF MEAT/CHICKEN, EGG ROLLS, AND SAUCES WERE SITTING OUT IN PANS IN THE KITCHEN/PREP AREA WAITING TO BE PUT IN THE COOLER. TO PREVENT BACTERIAL GROWTH, THESE NEED TO BE KEPT AT 41 DEGREES F OR LOWER AND PUT IN THE COOLER/FREEZER AFTER THEY ARE FINISHED BEING PREPPED.
  4. Critical, repeat violation: Refrigerated, ready-to-eat, TCS foods not properly date marked. THE EGG ROLLS AND COOKED CHICKEN IN THE REACH-IN-COOLER NEED TO BE DATEMARKED WITH A 7-DAY USE BY DATE.

Casa Express, 271 S. Sandusky St., Tiffin. Inspected Jan. 18, 2023:

  1. Critical, repeat violation: TCS foods not being hot held at the proper temperature. THE QUESO DIP AT THE BAR HAD AN INTERNAL TEMPERATURE OF 110 DEGREES F. TO PREVENT BACTERIAL GROWTH, HOT HOLDING NEEDS TO BE MAINTAINED AT 135 DEGREES F. THIS SHOULD BE HEATED ON THE STOVE BEFORE PLACING IN THE KETTLE, SO THAT IT IS AT THE REQUIRED TEMPERATURE BEFORE SERVING.
  2. Non-critical: Food stored in prohibited area. IN THE BACK, OUTSIDE HALLWAY, POP BOXES AND BOXES OF BAR MIX WERE BEING STORED. THIS IS STILL BEING EXPOSED TO THE WEATHER AND OUTSIDE.

McDonald’s, 1714 W. Market St., Tiffin. Inspected Jan. 18, 2023:

  1. Critical, corrected during inspection: Chlorine sanitizing solution was observed at incorrect temperature and/or concentration. THE DISH MACHINE DID NOT HAVE ANY SANITIZER. THE SANITIZER CANISTER WAS EMPTY. IT WAS REPLACED AND THEN THE MACHINE WAS OPERATING PROPERLY WITH CHLORINE SANITIZER AT 100 PPM.
  2. Non-critical: Non-food contact surfaces of equipment are unclean. INSIDE THE TRAULSEN REACH-IN-COOLER AND THE MCCAFE COOLER NEED TO BE CLEANED.
  3. Non-critical, repeat violation: Facility not maintained clean. THE FOLLOWING NEEDS TO BE CLEANED: UNDERNEATH THE DISHWASHING MACHINE, UNDER AND BEHIND THE ICE CREAM MACHINE, AND THE FLOORS THROUGHOUT.

Double Barrell Bar & Grill, 295 Vine St., Tiffin. Inspected Jan. 20, 2023:

  1. Critical, corrected during inspection: Improper packaging of food using reduced oxygen packaging without a variance. Please stop using the residential ROP machine for the steaks. The steaks need to be placed in a different type of container that doesn’t involve removing oxygen from the package. If you would like to ROP the steaks a commercial unit must be used and a HACCP plan must be submitted to our office for review and approval.

Fort Ball Pizza Palace (North Store), 91 N. Washington St., Tiffin. Inspected Jan. 23, 2023:

  1. Critical, repeat violation: Food not properly protected from contamination by separation, packaging, and segregation. OBSERVED CONTAINERS OF FOOD THAT WERE NOT COVERED. ALSO, IN THE PIZZA PREP COOLER, CONTAINERS OF FOOD WERE SITTING IN OTHER CONTAINERS OF FOOD. TO PREVENT CONTAMINATION, THE FOOD NEEDS TO BE KEPT COVERED AND CONTAINERS CANNOT SIT IN OTHER FOOD CONTAINERS.
  2. Critical, repeat violation: Refrigerated, ready-to-eat, TCS foods not properly date marked. IN THE BEER WALK-IN-COOLER, CONTAINERS OF MACARONI SALAD AND CUCUMBER SALAD WERE NOT DATEMARKED. IN THE BACK WALK-IN-COOLER, COOKED SAUSAGE WAS NOT DATEMARKED. THESE NEED TO BE DATEMARKED WITH A 7 DAY USE-BY DATE.
  3. Non-critical, repeat violation: Equipment not approved by a recognized testing agency. IN THE BACK WALK-IN-COOLER, MUSHROOMS WERE BEING KEPT IN A RUBBERMAID TOTE. THE TOTE NEEDS TO BE REPLACED WITH A COMMERCIAL-GRADE FOOD CONTAINER.
  4. Non-critical: Non-food contact surfaces of equipment are unclean. THE INSIDE AND OUTSIDE OF THE SUB PREP COOLER NEEDS TO BE CLEANED.
  5. Non-critical, repeat violation: Physical facilities not maintained in good repair. IN THE BACK PREP AREA, THE WOOD PANEL WALLS NEED TO BE REDONE SO THAT THEY ARE SMOOTH, CLEANABLE AND NON-ABSORBENT.

The Clover Club, 266 S. Washington St., Tiffin. Inspected Jan. 30, 2023:

  1. Critical: Improper bare-hand contact with RTE foods. OBSERVED BARE HAND CONTACT WHILE PREPARING WRAPS AND SANDWICHES.

Carmie’s / Camden Falls, 2460 S. SR 231, Tiffin. Inspected Feb. 1, 2023:

  1. Critical, repeat violation, corrected during inspection: Refrigerated, ready-to-eat, TCS foods not properly date marked. IN THE COKE REACH-IN-COOLER, A CONTAINER OF PICKLE SAUCE AND CHICKEN WINGS WERE DATEMARKED WITH A USE-BY DATE OF 1/31. BOTH WERE REMOVED.
  2. Non-critical, repeat violation: Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. THE UNDERCOUNTER COOLER DID NOT HAVE A THERMOMETER INSIDE.
  3. Non-critical, repeat violation: Non-durable equipment observed. THE SHELVES IN THE COKE COOLER AND PREP COOLER WERE RUSTY. THESE NEED TO BE SMOOTH AND CLEANABLE.
  4. Non-critical: No sanitizer test kit available. TEST STRIPS NEED TO BE PROVIDED FOR THE QUAT SANITIZER.
  5. Non-critical: Facility not maintained clean. THE WALL BEHIND THE FRYERS NEEDS TO BE CLEANED.
