Fostoria man dies, another hospitalized after vehicle crash: OSHP

Fostoria, Ohio — A Fostoria man has died and another was hospitalized with minor injuries following a two-vehicle crash Friday afternoon.

The crash occurred on Midblock Underpass, near W. Lytle Street, in the city of Fostoria. It was reported at approximately 3:54 p.m., according to the Findlay Post of the Ohio State Highway Patrol.

A red 2006 Mazda 3 was driven by 50-year-old Kevin Tomkinson, of Fostoria, southbound on Midblock Underpass.

A yellow 1997 Ford F450 box truck was driven by 78-year-old Dennis Debelak, of Fostoria, who was also southbound on Midblock Underpass.

Mr. Debelak was stopped at a red light when he was struck from behind by Mr. Tomkinson.

Mr. Debelak was wearing his safety belt and was taken to Promedica-Fostoria Hospital with minor injuries.

Mr. Tomkinson was not wearing a safety belt and was also taken to Promedica-Fostoria Hospital, where he was later pronounced deceased.

Assisting at the scene were Fostoria Police Department, Fostoria Fire & EMS, and D&D Towing.

Alcohol is believed to be a factor, and the crash remains under investigation.  The Ohio State Highway Patrol reminds motorists to never drive impaired and always wear your seat belt.
